BACKGROUND & AIMS: Genetic ancestry or racial differences in health outcomes exist in diseases associated with systemic inflammation (eg, COVID-19). This study aimed to investigate the association of genetic ancestry and race with acute-on-chronic liver failure (ACLF), which is characterized by acute systemic inflammation, multi-organ failure, and high risk of short-term death. METHODS: This prospective cohort study analyzed a comprehensive set of data, including genetic ancestry and race among several others, in 1274 patients with acutely decompensated cirrhosis who were nonelectively admitted to 44 hospitals from 7 Latin American countries. RESULTS: Three hundred ninety-five patients (31.0%) had ACLF of any grade at enrollment. Patients with ACLF had a higher median percentage of Native American genetic ancestry and lower median percentage of European ancestry than patients without ACLF (22.6% vs 12.9% and 53.4% vs 59.6%, respectively). The median percentage of African genetic ancestry was low among patients with ACLF and among those without ACLF. In terms of race, a higher percentage of patients with ACLF than patients without ACLF were Native American and a lower percentage of patients with ACLF than patients without ACLF were European American or African American. In multivariable analyses that adjusted for differences in sociodemographic and clinical characteristics, the odds ratio for ACLF at enrollment was 1.08 (95% CI, 1.03-1.13) with Native American genetic ancestry and 2.57 (95% CI, 1.84-3.58) for Native American race vs European American race CONCLUSIONS: In a large cohort of Latin American patients with acutely decompensated cirrhosis, increasing percentages of Native American ancestry and Native American race were factors independently associated with ACLF at enrollment.

INTRODUCTION: During the COVID-19 pandemic, endoscopic procedures are associated with a high risk of SARS-CoV-2 infection. However, in cases of upper gastrointestinal bleeding (UGIB), priority should be given to an early endoscopy. OBJECTIVE: The main objective was to compare the time since arrival at the hospital and the performance of the endoscopy between both groups. MATERIALS AND METHODS: We performed a retrospective study. Data contains information of patients who attended to the hospital with UGIB and underwent an endoscopy between October 19th, 2019 and June 6th, 2020. Patients were divided into 2 phases: pre-pandemic and pandemic. The time between arrival at the hospital and the performance of the endoscopy in both phases were compared as well as other indicators such hospital stay and in-hospital mortality. RESULTS: With information from 219 patients, the median age was 69 years. 154 and 65 endoscopies were performed in pre-pandemic and pandemic phase, respectively. The time between arrival at the hospital and the performance of the endoscopy was significantly longer during the pandemic (10.00 vs. 13.08 hours, p-value = 0.019). Nevertheless, there were no significant differences in hospital stay or mortality. CONCLUSION: The management of patients with UGIB during the COVID-19 pandemic is complex and requires the application of clinical judgment to decide the best timing to perform an endoscopy without affecting patient care.

The factors associated with an unfavorable evolution were the presence of diffuse peritoneal symptoms and a perforation size greater than or equal to 10 mm. CONCLUSION: endoscopic closure with endoclips is effective to avoid surgery in cases of deep mural injury or perforation after an endoscopic resection

INTRODUCTION: In 2018, the Clinical Practice Guideline (CPG) for the Evaluation and Management of upper gastrointestinal bleeding (UGB) was published by the Social Security of Peru (EsSalud). It provides evidence-based statements to optimize the management of these patients. OBJECTIVE: To evaluate the adherence to the statements of the CPG at the Edgardo Rebagliati Martins National Hospital (HNERM) of EsSalud (Lima, Peru). MATERIALS AND METHODS: Retrospective study, which analyzed the database of all patients who came to the HNERM emergency service with suspected UGB and were scheduled for endoscopy between October 19, 2019 and April 15, 2020. We included those with ≥ 18 years of age. This database contains the main characteristics of the standardized medical history for patients with UGB. Compliance with 13 of the 34 statements of the EsSalud CPG was evaluated. The results were presented descriptively, and the factors associated with compliance with the statements with insufficient adherence (<80%) and with more than 100 evaluated participants were evaluated. RESULTS: Data were obtained from 184 patients who met the inclusion criteria (men: 59.8%, median age: 70 years). The range of adherence to the 13 statements was from 63.2% to 99.5%. Only two statements had insufficient adherence (<80%). The statement with the least adherence was the recommendation to perform a restrictive transfusion. Noncompliance with this recommendation was found to be lower in those who had a higher score on the Glasgow-Blatchford index, a urea creatinine ratio > 60, and a lower hemoglobin on admission. CONCLUSION: Of the 13 statements evaluated, 11 had satisfactory adherence. It is important to explore the reasons why adherence is not adequate for some statements, and to evaluate methods to increase this adherence.

The objective of this study was to describe the clinical characteristics, management and mortality of adult patients hospitalized by COVID-19 during the first fortnight of May 2020 at the Edgardo Rebagliati Martins National Hospital (Lima, Peru). A retrospective cohort was performed reviewing electronic medical records. Data from 152 patients was collected (68.4% male, average age: 58.7 years). It was found that 27.0% had close contact with a person with COVID-19, 64.2% came to the hospital with a critical illness, 91.4% received hydroxychloroquine and 96.1% received azithromycin, 23.7% entered the intensive care unit, and mortality was 18.8%. In conclusion, more than half of the patients came to this hospital with a critical illness, almost all received drugs that were initially seen as potentially useful but are not currently recommended for the management of COVID-19 in hospitalized, and mortality was similar to that reported in other countries.


El presente estudio tuvo por objetivo describir las características clínicas, manejo y mortalidad de pacientes adultos hospitalizados por COVID-19 durante la primera quincena de mayo del 2020 en el Hospital Nacional Edgardo Rebagliati Martins (Lima, Perú). Se realizó una cohorte retrospectiva revisando historias clínicas electrónicas. Se recolectaron datos de 152 pacientes (68,4% varones, edad promedio: 58,7 años). Se encontró que 27,0% tuvo contacto cercano con una persona con COVID-19, 64.2% llegó al hospital con una enfermedad crítica, 91,4% recibió hidroxicloroquina y 96,1% recibió azitromicina, 23,7% ingresó a la unidad de cuidados intensivos, y la mortalidad fue de 18,8%. En conclusión, más de la mitad de los pacientes acudieron a este hospital con una enfermedad crítica, casi todos recibieron fármacos que inicialmente fueron vistos como potencialmente útiles pero que actualmente no son recomendados para el manejo de COVID-19 en hospitalizados, y la mortalidad fue similar a lo reportado en otros países.

RESUMEN Introducción : Durante la pandemia de COVID-19, los procedimientos endoscópicos se asocian con un alto riesgo de infección por SARS-CoV-2. Sin embargo, en casos de hemorragia digestiva alta (HDA), se debe dar prioridad a una endoscopia precoz. Objetivo : El objetivo principal fue comparar el tiempo transcurrido desde la llegada al hospital y la realización de la endoscopia entre ambos grupos. Materiales y métodos : Realizamos un estudio retrospectivo. Los datos contienen información de pacientes que acudieron al hospital con HDA y fueron sometidos a endoscopia entre el 19 de octubre de 2019 y el 6 de junio de 2020. Los pacientes se dividieron en 2 fases: prepandémica y pandémica. Se comparó el tiempo transcurrido entre la llegada al hospital y la realización de la endoscopia en ambas fases, así como otros indicadores como la estancia hospitalaria y la mortalidad intrahospitalaria. Resultados : Con información de 219 pacientes, la mediana de edad fue de 69 años. Se realizaron 154 y 65 endoscopias en fase prepandémica y pandémica, respectivamente. El tiempo entre la llegada al hospital y la realización de la endoscopia fue significativamente mayor durante la pandemia (10,00 frente a 13,08 horas, valor de p = 0,019). Sin embargo, no hubo diferencias significativas en la estancia hospitalaria ni en la mortalidad. Conclusión : El manejo de pacientes con HDA durante la pandemia de COVID-19 es complejo y requiere la aplicación del juicio clínico para decidir el mejor momento para realizar una endoscopia sin afectar la atención del paciente.

Acute pancreatitis (AP) is an acute inflammatory process of the pancreas that, according to the 2012 Atlanta classification, can be mild, moderate or severe. OBJECTIVE: Describe the epidemiological, clinical, therapeutic and outcomes of patients with AP admitted to the intensive care and intermediate care unit of a general hospital and compare them with those described in the national and international literature. MATERIALS AND METHODS: Observational study of patients with AP treated over a period of 3 years. RESULTS: 59 cases were included; the female sex prevailed (54.2%), the average age was 59.3 years, the most frequent etiology was biliary (84.7%). Average entry severity scores were APACHE II of 12.4 points, SOFA of 4.9 points and Marshall modified of 2.8 points; The most frequent organ failure was respiratory (47.5%). The average stay in care was 13.9 days and in the hospital it was 23.3 days. No patients with mild or moderate AP died during their hospital stay, 6 patients with severe AP died during their hospital stay (20% of cases of severe AP). CONCLUSION: The cases of our hospital had a clinical and therapeutic profile similar to that described in the world and Latin American literature. A hospital staying was greater than that described in recent works, but our mortality was lower.

RESUMEN Introducción : El año 2018 se publicó la Guía de práctica clínica (GPC) para la evaluación y el manejo de la hemorragia digestiva alta (HDA) del Seguro Social de Salud del Perú (EsSalud). Esta emite enunciados basados en evidencias, que buscan optimizar el manejo de estos pacientes. Objetivo : Evaluar la adherencia a los enunciados de dicha GPC en el Hospital Nacional Edgardo Rebagliati Martins (HNERM) de EsSalud (Lima, Perú). Materiales y métodos : Estudio retrospectivo, que analizó la base de datos de todos los pacientes que acudieron al servicio de emergencia del HNERM con sospecha de HDA y fueron programados para endoscopía entre el 19 de octubre del 2019 y el 15 de abril del 2020. Se incluyó a aquellos con ≥ 18 años de edad. Esta base contiene las principales características de la historia clínica estandarizada para pacientes con HDA. Se evaluó el cumplimiento de 13 de los 34 enunciados de la GPC de EsSalud. Los resultados se presentaron descriptivamente, y se evaluaron los factores asociados al cumplimiento de los enunciados con inadecuada adherencia (< 80%) y con más de 100 participantes evaluados. Resultados : Se obtuvieron datos de 184 pacientes que cumplieron con los criterios de inclusión (varones: 59,8%, mediana de edad: 70 años). El rango de adherencia a los 13 enunciados fue de 63,2% a 99,5%. Sólo dos enunciados tuvieron adherencia inadecuada (<80%). El enunciado con menor adherencia fue la recomendación de realizar transfusión restrictiva. Se encontró que el cumplimiento de esta recomendación fue menor en quienes tuvieron mayor puntaje en el índice de Glasgow-Blatchford, relación urea creatinina > 60, y menor hemoglobina al ingreso. Conclusión : De los 13 enunciados evaluados, 11 tuvieron una adherencia adecuada. Resulta importante profundizar en los motivos por los cuales la adherencia no es adecuada para algunos enunciados, y valorar estrategias para aumentar esta adherencia.

RESUMEN Introducción. La neoplasia mucinosa papilar intraductal (IPMN) se diagnostica ahora con mayor frecuencia debido al mayor uso de los métodos de imágenes, y conlleva un desafío clínico su manejo y seguimiento por su probable transformación maligna. Objetivos. Conocer las características por ecoendoscopía (USE), evolución clínica y sobrevida de los pacientes diagnosticados de IPMN por USE. Materiales y métodos. Estudio de cohorte retrospectivo con análisis de sobrevida en pacientes diagnosticados de IPMN por USE entre 2013 y 2018 en el Hospital Nacional Edgardo Rebagliati Martins (HNERM). Se analizaron variables demográficas y ecoendoscópicas, además de seguimiento hasta el fallecimiento o 6 meses después del término del estudio. Se realizó el análisis de sobrevida con los métodos de Kaplan-Meyer y de regresión de Cox. Resultados. Se incluyeron 133 pacientes con IPMN. Edad media fue 68,6 años, 80 (60,2%) fueron mujeres. Según subtipos, 89 (66,9%) fueron de rama secundaria, 23 (17,3%) de ducto principal y 21 (15,8%) de tipo mixto. La principal localización fue cabeza de páncreas (41,4%). En el seguimiento, 22 (16,5%) fueron a cirugía, 22 (16,5%) fallecieron después de una mediana de seguimiento de 522 días. En 8 pacientes (6%) se detectó neoplasia maligna. La tasa de sobrevida global fue 86.8% (IC 95%, 79,6-91,6) al año y de 81.9% (IC95%, 73,3-88,0) a los 3 años. En análisis univariado los factores asociados a la sobrevida fueron los subtipos de IPMN-DP (p=0,02) y mixto (p=0,005), sexo masculino (p=0,004), tamaño de lesión ≥30 mm (p=0,000), nódulos (p=0,014) y Wirsung ≥10 mm (p=0,01). En el análisis multivariado, los factores predictores asociados con la sobrevida fueron: IPMN-DP (HR=6,3, p=0,005), IPMN mixto (HR=4,9, p=0,008) y tamaño de lesión ≥30 mm (HR=7,1, p=0,000). Conclusiones. El diagnostico de IPMN de ducto principal y mixto se asocian como factores predictores de sobrevida, al igual que el tamaño de la lesión ≥30 mm.


ABSTRACT Introduction: Intraductal papillary mucinous neoplasms (IPMN) are diagnosed more frequently because the higher use of radiologic exams, in that sense they are a great challenge to define its management and treatment in relation to its potential malignant transformation. Objective: To describe IPMN clinical profile, endoscopic ultrasound (EUS) characteristics and survival in all patients diagnosed with IPMN by EUS at HNERM. Materials and methods: Retrospective cohort of patients with IPMN diagnosed at HNERM by EUS from 2013 to 2018. Descriptive statistics was used for clinical profile and EUS characteristics. Kaplan Meir Method and Cox regression analysis was applied for survival analysis. Results: 133 patients with IPMN were included. Medium age was 68.6 years, 80 (60.2%) were female. According to IPMN subtypes, 89 (66.9%) originated from secondary branch, 23 (17.3%) from main duct (MD) and 21 (15.8%) were mixed type (MT). Head of pancreas was the main localization (41.4%). In follow-up, 22 (16.5%) were derived to surgery. Mortality occurred in 16.5% (22 cases) after a median follow-up of 522 days. Malignant transformation was diagnosed in 6% (8 cases). Survival was 86.8% (IC 95%, 79.6-91.6) at 1 year and 81.9% (IC95%, 73.3-88.0) at 3 years. Univariate analysis demonstrated that factors associated to survival were MD-IPMN (p=0.02) y MT-IPMN (p=0.005), male gender (p=004), nodule size ≥30 mm (p=0.000), presence of nodules (p=0.014) and Wirsung ≥10 mm (p=0.01). Multivariate analysis showed that predictive factors for survival were MD-IPMN (HR=6.3, p=0.005), MT-IPMN (HR=4.9, p=0.008) and nodule size ≥30 mm (HR=7.1, p=0.000). Conclusions: Diagnosis of MD-IPMN and MT-IPMN are predictive factors for survival as well as nodule size ≥ 30mm.

BACKGROUND: In recent years there has been an increase in the use of self-expanding metallic stents (SEMS) in patients with unresectable malignant stenosis of the bile duct. However, in Peru there are no reports on the safety and efficacy of this procedure. OBJECTIVE: To evaluate the safety and efficacy of the use of SEMS in the palliative management of malignant stenosis of the bile duct. MATERIALS AND METHODS: Retrospective cohort. All patients referred for placement of biliary SEMS as part of a palliative treatment between January 2016 and August 2018 were included. Rates of successful placement of SEMS, adequate palliation of the obstruction, and complications associated with the procedure were obtained. The patency of the prosthesis was evaluated during follow-up. Survival was determined after placement of the prosthesis. RESULTS: We included 32 patients with indication of palliative management due to an unresectable malignant stenosis of the bile duct. Pancreatic cancer (56.25%) followed by cholangiocarcinoma (31.25%) were the most frequent etiologies. A successful first-time placement rate of 96.97% was achieved. Adequate palliation of biliary obstruction was achieved in 100% of patients (p<0.05). Two SEMStc migrated during follow-up (6.25%) being managed with the placement of a new SEMSuc. CONCLUSIONS: The placement of SEMS is a safe and effective therapeutic strategy in the palliative management of patients with malignant obstruction of the biliary tract.

OBJECTIVE: The ideal therapy for chronic hepatitis C is the use of direct acting antivirals (DAA). In Peru there is no data in this aspect, in that sense it is necessary to report real life experience with these drugs. MATERIAL AND METHODS: A digital survey was sent through e-mail to hepatologists, and the data of four is considered in this study. Statistical analysis was descriptive. RESULTS: We included 63 patients, mean age was 59 years, 49.21% were male, cirrhosis was present in 49.21%, and 34.92% was non-responder to PEGIFN and Ribavirin. Genotype 1 was present in 93.65%, and subtype 1a was 58.73%, there were only 2 cases with Gt 2 and one with Gt 3. There were 10 different DAA combinations used, and the most effective were Sofosbuvir/Ledipasvir, Sofosbuvir/Ledipasvir/Ribavirina and Sofosbuvir/Simeprevir, in all these cases the Sustained Viral Response (SVR) was 100%. The other combinations had SVR < 90% or only 1-2 patients included. All patients tolerated treatments and no serious adverse events occurred. CONCLUSIONS: In real life antiviral treatment for hepatitis C with AAD is effective and well tolerated. The best SVR was obtained with Sofosbuvir/Ledipasvir, Sofosbuvir/Ledipasvir/Ribavirina and Sofosbuvir/Simeprevir. This report may be useful to consider treatment strategies with focus in public health.

Antecedentes: En los últimos años se ha producido un incremento del uso de prótesis metálicas autoexpandibles (SEMS) en pacientes con estenosis malignas irresecables de la vía biliar. Sin embargo, en Perú no se cuentan con reportes sobre la seguridad y eficacia de este procedimiento. Objetivo: Evaluar la seguridad y eficacia del uso de las SEMS en el manejo paliativo de las estenosis malignas de la vía biliar. Materiales y métodos: Cohorte retrospectiva. Se incluyeron a todos los pacientes referidos para colocación de SEMS biliar como parte de un tratamiento paliativo entre enero del 2016 y agosto del 2018. Se obtuvieron las tasas de colocación exitosa de las SEMS, de paliación adecuada de la obstrucción y de complicaciones asociadas al procedimiento. Se evaluó la patencia de la prótesis durante el seguimiento. Se determinó la supervivencia luego de la colocación de la prótesis. Resultados: Se incluyeron 32 pacientes con indicación de manejo paliativo debido a una estenosis maligna irresecable de la vía biliar. El cáncer de páncreas (56,25%) seguido del colangiocarcinoma (31,25%) fueron las etiologías más frecuentes. Se alcanzó una tasa de colocación exitosa en primera intención de 96,97%. La paliación adecuada de la obstrucción biliar se alcanzó en el 100% de los pacientes (p<0,05). Dos SEMStc migraron durante el seguimiento (6,25%) siendo manejados con la colocación de una nueva SEMSnc. Conclusiones: La colocación de SEMS constituye una estrategia segura, con alta tasa de éxito terapéutico en el manejo paliativo de los pacientes con obstrucción maligna de la vía biliar.

Objetivos: La terapia ideal para la hepatitis crónica C consiste en el uso de drogas antivirales de acción directa (DAA). En el Perú la experiencia en vida real con DAA no se conoce, por lo que el objetivo del presente estudio es reportar la alta eficacia terapéutica con estos esquemas. Material y métodos: Mediante correo electrónico se invitó a participar a través de una encuesta a médicos hepatólogos a nivel nacional. Se incluyeron los datos de 4 médicos. Los resultados fueron analizados con estadística descriptiva. Resultados: Se incluyeron 63 pacientes, la edad promedio fue 59 años, varones fueron 49,21%, cirrosis estuvo presente en el 49,21% (31/63), 34,92% había sido no respondedor a terapia con PEGIFN/RBV. El Genotipo 1 estuvo presente en 93,65% de casos, siendo el 1a el predominante (58,73%). Solo hubo dos casos de genotipo 2 y uno de genotipo 3. Se utilizaron 10 esquemas de combinación con DAA, siendo los más eficaces, Sofosbuvir/Ledipasvir, Sofosbuvir/Ledipasvir/ Ribavirina y Sofosbuvir/Simeprevir, en todos ellos se logró Respuesta Viral Sostenida (RVS) de 100%. Con los otros 7 esquemas la RVS fue menor a 90% o solo se había incluido uno o dos pacientes. La tolerancia a la terapia fue adecuada y todos los pacientes culminaron la terapia. Conclusiones: En vida real los esquemas de terapia antiviral para hepatitis C con DAA tienen alta eficacia y seguridad. Las mejores respuestas se obtuvieron con Sofosbuvir/Ledipasvir, Sofosbuvir/Ledipasvir/Ribavirina y Sofosbuvir/Simeprevir. Esta data puede ser útil para considerar estrategias de tratamiento con el enfoque de salud pública.

BACKGROUND: endoscopic dilation is considered as the treatment of choice for esophageal strictures. However, there are no studies in our region that have assessed the safety of the procedure. OBJECTIVE: to assess the safety of esophageal dilation and the factors associated with the development of complications. MATERIALS AND METHODS: a retrospective cohort was studied. All patients referred for esophageal dilation between January 2015 and June 2017 were included in the study. A complication rate was obtained and the association between nonadherence to the "rule of 3" and the development of complications was determined. Other predictive factors associated with complication development were also analyzed. RESULTS: a total of 164 patients that underwent 474 dilations were included in the study. Surgical anastomosis stricture was the most prevalent etiology. A total of six complications occurred, including three perforations (0.63%), two bleeding events (0.42%) and one episode of significant pain that required post-procedure observation (0.21%). Endoscopic esophageal dilation without adherence to the "rule of 3" was not associated with a higher risk of complications. Balloon dilation was the only predictive factor for complications. CONCLUSIONS: esophageal dilation is a safe procedure. Nonadherence to the "rule of 3" does not appear to be associated with a higher risk of complications, including esophageal perforation.

Antecedentes: las dilataciones endoscópicas se consideran el tratamiento de elección para las estenosis esofágicas, sin embargo, no se cuenta en nuestra región con estudios en los que se evalúe la seguridad de este procedimiento. Objetivo: evaluar la seguridad de las dilataciones esofágicas y los factores asociados a las complicaciones. Material y métodos: cohorte retrospectiva. Se incluyeron todos los pacientes referidos para dilatación esofágica entre enero de 2015 y junio de 2017. Se obtuvo la tasa de complicaciones y se determinó la asociación entre la no adherencia a la "regla de los tres" y el desarrollo de complicaciones. Se analizaron otros factores predictivos asociados al desarrollo de complicaciones. Resultados: se incluyeron 164 pacientes en los que se realizaron 474 dilataciones. La etiología más prevalente fue la estenosis de anastomosis quirúrgica. Se presentaron seis complicaciones: tres perforaciones (0,63%), dos sangrados (0,42%) y un episodio de dolor significativo que requirió observación posprocedimiento (0,21%). La dilatación endoscópica esofágica fuera de la "regla de los tres" no se encontró asociada a un mayor riesgo de complicaciones. La dilatación con balón fue el único factor predictivo para el desarrollo de complicaciones. Conclusiones: la dilatación esofágica es un procedimiento seguro. La no adherencia a la "regla de los tres" no parece estar asociada a un mayor riesgo de complicaciones, incluyendo la perforación esofágica

Objetivo: Describir la experiencia clínica con la técnica de dilatación de la esfinterotomía papilar con balones de gran diámetro en pacientes con coledocolitiasis de difícil extracción. Materiales y métodos: Estudio retrospectivo, diseño descriptivo. Serie de Casos. Se analizaron las historias clínicas de 18 pacientes que fueron sometidos a colangiopancreatografía retrograda endoscópica (CPRE) más dilatación papilar con balón de gran diámetro (DPBGD) por presentar coledocolitiasis de gran tamaño (≥15 mm), desproporción de diámetro entre cálculo y colédoco distal y/o papila yuxtadiverticular. Se emplearon balones dilatadores CRETM entre 12 y 20mm de diámetro. Se consignaron datos como éxito del procedimiento, uso de litotricia; así como complicaciones durante el procedimiento. Resultados: La edad promedio fue 66,1 años. Hubo predominio del género femenino (66,7%). El tamaño promedio de los cálculos en vía biliar fue de 16,7 mm. Las indicaciones de DPBGD fueron: coledocolitiasis gigante (12 pacientes, 66,7%), discordancia entre el diámetro del cálculo y el colédoco distal (6 pacientes, 33,3%). El diámetro de los balones de dilatación más frecuentemente empleados fueron: 15 mm (8 pacientes, 44,4%), 18 mm (5 pacientes, 27,8%), 12 mm (3 pacientes, 16,7%) y 20 mm (2 pacientes, 11,1%). Se consiguió la extracción completa de los cálculos en 15 pacientes (83,3%). Se precisó litotricia en 4 pacientes (22,2%). Hubo 3 pacientes en los que la extracción con balón fue frustra, realizándose manejo quirúrgico. Se reportó 1 caso de pancreatitis aguda leve (5,5%). Conclusiones: Los resultados demuestran que la dilatación con balón es una alternativa segura y eficaz en el manejo de los cálculos en vía biliar de difícil extracción


Objective: The aim of this study was to report the initial experience of the combined use of biliary sphincterotomy plus balloon dilatation of the papilla for management of large stones. Materials and methods: Design: Retrospective, descriptive. This study included 18 patients in whom a hydrostatic dilatation of the papilla with large balloons was performed between June 2012 and April 2014. Patients had multiple large stones, tapered distal common bile duct, previous sphincterotomy, or peri/ intradiverticular papilla. CRE™ dilatation balloons with diameters ranging from 12 to 20 mm were used. Data were recorded as successful procedure, use of lithotripsy and complications during the procedure. Results: The average age was 66.1 years. There was a predominance of the female gender (66.7%). The average size of the bile duct stones was 16.7 mm. The main indications were: giant choledocholithiasis (12 patients, 66.7%) and tapered distal common bile duct (6 patients, 33.3%). The dilatation balloons diameter used were: 15 mm (8 patients, 44.4%), 18 mm (5 patients, 27.8%), 12 mm (3 patients, 16.7%) and 20 mm (2 patients, 11.1%). Complete stone clearance was achieved in 15 patients (83.3%). Lithotripsy was performed in 4 patients (22.2%). There were 3 patients in whom the removal with balloon was unsuccessful, performed surgical management. It was reported 1 case of mild acute pancreatitis (5.5%). Conclusions: The results show that endoscopic papillary large balloon dilation after sphincterotomy is a safe and effective technique for treatment of difficult bile duct stones

OBJECTIVE: To determine the prevalence of Helicobacter pylori in symptomatic outpatients in Red Rebagliati (EsSalud) from 2010 to 2013. MATERIAL AND METHODS: An observational, transversal and analytical study. Records of outpatients with diagnostic 13C-urea breath test of Helicobacter pylori in the period 2010-2013 were reviewed. RESULTS: Of the 1711 patients, the overall prevalence of Helicobacter pylori was 45.5% (95% CI: 43.17-47.89) being higher in women than in men (47.1% vs. 42.1%, p=0.056). There were significant differences in age between infected and uninfected (44 vs. 39, p < 0.05), with association between age and the presence of Helicobacter pylori (p<0.001). The estimated prevalence in the pediatric population was 36.3% and was found positive in 201 (51.1%) women of childbearing age. Most (43.9%) came from the financial sector of the city. CONCLUSIONS: The prevalence of Helicobacter pylori in the study population was similar in both sexes and is related to age. The average socioeconomic status of Lima maintains the prevalence of Helicobacter pylori reported in the last years.

Determinar la prevalencia de Helicobacter pylori en pacientes sintomáticos de consulta externa en la Red Rebagliati (EsSalud) en el período 2010-2013. Materiales y métodos: Estudio observacional, transversal y analítico. Se revisaron los registros de pacientes ambulatorios con la prueba en aliento con urea-13C diagnóstica de Helicobacter pylori en el período 2010-2013. Resultados: De los 1711 pacientes, la prevalencia global de Helicobacter pylori fue 45,5% (IC 95%: 43,17- 47,89), siendo mayor en mujeres que en varones (47,1% vs. 42,1%, p=0,056). Hubo diferencias significativas en la edad entre infectados y no infectados (44 vs. 39, p<0,05), con asociación entre la edad y presencia de Helicobacter pylori (p<0,001). La prevalencia estimada en la población pediátrica fue 36,3% y se encontró resultado positivo en 201 (51,1%) mujeres en edad fértil. La mayoría (43,9%) procedían del sector financiero de la ciudad. Conclusiones: La prevalencia de Helicobacter pylori en la población de estudio fue similar en ambos sexos y tuvo relación con la edad. El nivel socioeconómico medio de Lima mantiene la prevalencia de Helicobacter pylori reportada en los últimos años...


To determine the prevalence of Helicobacter pylori in symptomatic outpatients in Red Rebagliati (EsSalud) from 2010 to 2013. Material and methods: An observational, transversal and analytical study. Records of outpatients with diagnostic 13C-urea breath test of Helicobacter pylori in the period 2010-2013 were reviewed. Results: Of the 1711 patients, the overall prevalence of Helicobacter pylori was 45.5% (95% CI: 43.17-47.89) being higher in women than in men (47.1% vs. 42.1%, p=0.056). There were significant differences in age between infected and uninfected (44 vs. 39, p<0.05), with association between age and the presence of Helicobacter pylori (p<0.001). The estimated prevalence in the pediatric population was 36.3% and was found positive in 201 (51.1%) women of childbearing age. Most (43.9%) came from the financial sector of the city. Conclusions: The prevalence of Helicobacter pylori in the study population was similar in both sexes and is related to age.
